Three Step Application Process

Several questionnaires and forms are important components of the services I offer. Once we decide what work we will do together, either assessment or therapy, I will let you know which of these questionnaires and forms to fill out. You can contact me to confirm the completion of all required forms & questionnaires. 

Step One: Read My Services

Please make sure you have read the relevant services pages in full. They will answer many of your questions. Additionally, they will explain the benefits, objectives and processes of the service. They will make clear what the service involves.

Step Two: Forms & Questionnaires

You will find the forms and questionnaires listed below. Additionally, you will see them listed in the title image above.

Complete the Following two Online Questionnaires for an ASD Assessment

Instructions: Click on each of the 2 links below. Clicking on a link will open the questionnaire in a new browser tab. You will see the corresponding questionnaire. Please complete both of them. You will receive email notification of completion.

Download and read Office Policies and General Information. Download and read the Informed Consent for Telehealth if we are meeting remotely. Fill out the Release of Information form once we have started an ASD assessment.
